		<description><![CDATA[The problem with these reports is the tendency to find culprits, instead of making an holistic approach and try to make useful recommendations in order to avoid re-occurrence. Issues like pilot training, retirement age of pilots, commercial pressure from owner and/or port authorities and the worldwide lowering of ship&#8217;s crew quality should be addressed. 
The lack of international common standards for pilot certification, training and recruitment is also unexplainable. IMO should take a proactive approach on this subject and enforce STCW-like regulations for pilots. 
It is not necessary to come up with new ideas, just look at what the aviation industry has accomplished in the last decades regarding the training and certification of airline pilots.

We know of at least two, the one the Cosco Busan followed  and the one evidently followed by all the other vessels that made it sea without getting in the news. The planned route, which existed only in the pilots head, should have been made know in some concrete form to the bridge team so that they could have monitored the vessels progress. It doesn&#039;t matter how many officers are on the bridge if none of them knows what  the pilot intents to do. ]]></description>
