Incident Picture of the Week - M/V Tampa Maersk

How did this happen? According to Cargo Law:

January 2000 the vessel was attempting to turn in the area in front of the Maersk Container Terminal, Oakland so as to berth port side to the dock and have the vessel facing outbound.
