The Marine Safety Forum gives us information on this lifeboat air bottle explosion:
A gas bottle exploded in a lifeboat of a vessel during the process of charging it up from the
Breathing Apparatus Air Compressor.
The Master of the 8 years-old vessel was in proximity of the lifeboat and he was very seriously
wounded. The bottle was quite old with different numbers/dates.
The photographs below speak for themselves. Unfortunately one person was injured – imagined
the results if this happened on the deck area with more people around.

Bob Couttie of Maritime Accident Casebook is giving MAIIF a helping hand researching accidents in confined spaces and is looking for your help. He writes:
I know there are more incidents out there that may not have been reported, or not well reported, either in the media or in official investigations by certain reluctant flag states, and there will be a lot of ‘close call’ incidents that don’t get on the radar. Do use the confidential@maritimeaccident.org if you, or someone you know, has an incident to report, or a comment on a specific incident or close call. Identities will be stripped out of the reports to protect the reporter.

Have an Iridium, VHF or SSB
handheld radio aboard? What about a portable GPS
? Create an Abandon ship electronics grab bag that is waterproof, floats and will survive a fire.
Here’s what you need to do:
- Buy a Pelican Case

- Stencil it with the ship’s Name
- Apply reflective SOLAS tape

- Laminate copies of the electronics’ manual
- Print and Laminate a list of emergency contact numbers
That’s pretty much it.
(This tip was suggested by Jean Pierre de Lutz. in his post “Sailing in Severe Weather Lessons Learned“.)
