NTSB Report

Meanwhile, the NTSB released its highly anticipated preliminary report on the accident. The report revealed that the ship experienced a series of four blackouts starting 10 hours before its departure when a crew member mistakenly closed an engine damper while performing maintenance on the ship’s scrubber system.

While still in port, the crew restore power and change configurations to use different breakers and transformers. However, the those same breakers tripped “unexpectedly” when the ship was only 0.6 miles from the Key Bridge, causing the vessel to lose propulsion and steering.

Although the Dali’s crew briefly restored electrical power, a second blackout occurred when the ship was only 0.2 miles away from the bridge, leading to a total power outage. Despite efforts to regain power and drop the anchor, the crew could not regain control, and the ship hit the bridge at a speed of 6.5 knots. What exactly led those breakers to trip is still under investigation.

The report dispels speculation that poor fuel quality may have contributed to the accident, stating: “Test results did not identify any concerns related to the quality of the fuel.”
