Default Service letter for DE

Does anyone have or know of a form letter suitable for application as a Designated Examiner?

Send in a letter that details your license and experience in the towing industry, and your desire to be appointed as a DE. It doesn't need to be much more than a paragraph. Generally, they want to see three years of master's time on tugs, although there is nothing in the NVIC about that.
